- Medical Records Send Request
– Complete this form to obtain a copy of your Umpqua Orthopedics medical records. As per OAR 847-012-000, medical records requests will be completed within 30 days from the date the signed request is received in our office. You may be charged a reasonable fee for reproducing your medical records, and payment is due in advance.
- Medical Records Obtain Request – to obtain records from another physician / facility (Save file in PDF format and email completed forms to info@umpquaortho.com)
Patient Forms Instructions
In order to enter your information into the patients form above, you would need to download the form to your Phone or PC. Once Downloaded, you will need to use a PDF editing software such as the Adobe app to make changes to the Forms.
Once Completed, save the Edited form with the changes, and submit via the website.
